What AhaCare is
Non-medical home care: help with personal routines, companionship, mobility and daily living, settling in after a hospital stay, and relief for a family caregiver.
One coordinator per family, and a caregiver the family meets before care starts.
Not skilled nursing, not therapy, and not a medical service. If that is what your patient needs, we are the wrong referral and will say so.
How to refer someone
Step 1. Send what you can
The same request form families use. A city, what help is needed, and a way to reach the family is enough to start.
Step 2. Leave the medical detail out
Please don't include diagnoses, medications, medical records, or other sensitive medical information in this form. A care coordinator can discuss relevant care needs with you directly.
Step 3. A coordinator takes it from there
They contact the family, work out whether we are the right fit, and tell you honestly if we are not.
